Does it matter if the rear brake drum/sprocket is on the wheel when I get it balanced?

i would have it on yes personally, but i don't think mine goes fast enough to even worry about balancing LOL

No, you don't. It is so close to the hub it will not affect the balance.

I've had them balanced both ways, and I could not tell any difference, even at highway speeds.

I've never even had the sprocket carrier on the rear wheel for balancing. Something as small as that would have to be so far off to make any difference.

Look into dynabeads. I use them on all my bikes. A little more costly, but no ugly wheel weights.

Don't hear them at all, they are tiny, and not much weight is added.
